* Price includes a $50 late registration fee.
**Reduced registration is available for postdoctoral fellows, trainees, and
nurses. When you check-in on site you will be required to show either 1. an
institutional ID badge showing your position or 2. a letter from your program
director stating that you are a fellow, trainee or nurse.

Gala Event Tickets:
The Gala Event dinner will be held at the Union Club of Boston on
Monday, June 11 from 6pm to 9pm. Ticket includes a cocktail hour and dinner.

CME Credits: This activity has been planned and implemented in accordance
with the Essential Areas and policies of the Accreditation Council for
Continuing Medical Education through the joint sponsorship of Beam Institute
and Shwachman-Diamond Syndrome Foundation. Beam Institute is accredited by the
ACCME to provide continuing medical education for physicians.
The Beam Institute designates this educational activity for a maximum of 16 AMA
PRA Category 1 Credit(s)™. Physicians should only claim credit commensurate
with the extent of their participation in the activity.
Physicians not licensed in the United States who participate in this CME
activity are eligible for AMA PRA Category 1 Credit(s)™.

Call for Abstracts
The Scientific Committee is now accepting abstracts for Oral and Poster
Presentation. Abstracts of no more than 300 words must be submitted online in
PDF form on or before March 31, 2007. Abstracts will be
selected by early May and the committee will contact all submissions by May 15.
Please use Times New Roman 12 point font (or equivalent) and include:

name of the person who will present the abstract
-
affiliation, e-mail address, telephone and fax number of the person presenting
the abstract
-
all authors of the abstract (Underline the name of speaker)
-
affiliations of the authors of the abstract
-
title of the abstract
-
text of the abstract
-
3 keywords applying to the abstract
-
Note if abstract is to be considered for oral and/or poster presentation. If
not specified, abstract will be considered for both.
Please note:
- The abstract should address scientific questions or clinical observations
related to Shwachman-Diamond Syndrome or similar conditions.
- There is no limit to the number of abstracts an individual may submit.
- You may submit an abstract during the registration process, or you may email
an abstract directly to Elise
Porter for consideration. Please note, you must still follow the
requirements above.

Travel & Accommodations:
The Congress will begin at approximately 8am on Sunday, June 10 and will end in
the early afternoon on Tuesday, June 12. Therefore, we recommend you plan to
arrive in Boston in the evening of Saturday, June 9 and to depart in the late
afternoon, Tuesday, June 12. If you need help in arranging your travel we can
recommend a local travel agent, Stewart International Travel (617-738-1575).
Hotel space for the meeting is limited. June is a very popular time to travel to
Boston and hotels fill quickly. It is strongly recommended you make your
reservations as soon as possible to ensure a hotel room for the congress. We
have a limited number of hotel rooms available at a reduced rate at the Inn at
Longwood and the Holiday Inn. These rooms are available on a first come first
serve basis. Please call the hotels directly and ask to reserve a room with the
SDS Congress block.
